Legal & General Group Protection has partnered with long-term condition management support service CONNECTPlus, from Health and Care Innovations Limited (HCI).
The evidence-based app, according to L&G, will play a key role in the insurer’s tailored multidisciplinary team (MDT) paths, assisting individuals in taking control of their conditions and improving their health – both at home and at work.
CONNECTPlus, which is available to all covered employees and their immediate families, aids in the self-management of the following six long-term conditions: stroke, cancer, multiple sclerosis, type 2 diabetes, long-term care, and rheumatoid arthritis. According to Legal & General, bringing a variety of ailments together in one area also helps address the needs of those who have more than one long-term condition.
CONNECTPlus was developed in collaboration with NHS physicians and patients, and is now in use in a number of NHS Foundation Trusts, with the goal of providing individuals with the tools they need to actively manage their own care. Increased control, time and money savings, less worry, and improved outcomes are all potential benefits of self-management.
According to L&G, the app also aids line managers and HR by assisting them in better understanding a condition’s impact on life and work capacity, allowing them to better support their staff. This includes, if necessary, making informed reasonable adjustments in accordance with the Equality Act 2010.
